FEVAR & FIE
The establishment of this individual record of exposure (FIE) to hazardous chemicals is a regulatory requirement.
This form can be useful in the future if you develop a disease that may be considered as an occupational disease.
The tool FEVAR will allow you to create or update your individual record of exposure to hazardous chemicals ( FIE) with FEVAR app.

You mustn’t rename the “FEVAR.xls” Excel file (with macros).
Fill in FEVAR, generate your exposition form in pdf format (the file must be titled “FEVAR_LastName_FirstName.pdf”) and send it to ibmp-ap@unistra.fr by January 31, 2022 at the latest. The email must have the subject “FEVAR”
The Prevention Assistants will collect and archive all the FIE and then forward them to prevention doctors and to the prevention services of CNRS and Unistra in February.
